Ovo je ujedno prvi put u povijesti Silent Hill serijala da idemo u Japan. Priča prati već spomenutu Shimizu Hinako, koja se teško nosi s očekivanjima koja od nje imaju obitelj, prijatelji i društvo. Sve se zakomplicira kada se nad njenim gradom spusti gusta magla i sa sobom donese čudna stvorenja.
Silent Hill f će biti rađen kao horor avantura s elementima preživljavanja. Imat ćemo pogled iz trećeg lica i koristit ćemo hladna oružja. Oni silni cvjetni motivi koji su nas privukli u prvih nekoliko pogleda na ovaj naslov su povezani s novim paralelnim svijetom pod nazivom Dark Shrine. Silent Hill f će imati jako snažnu poruku koju će nositi, a odnosi se na rodne diskriminacije, maloljetničko nasilje, zlostavljanje djece, halucinacije izazvane drogama, pa čak i mučenje.
