Mafia: The Old Country je najnoviji naslov u Mafia serijalu koji će igrača upoznati s Enzom, mladim Talijanom koji od malena radi u rudniku sumpora. Enzu se tada ukaže prilika da se pridruži familiji Dona Torrisija, koju mladić prihvaća. Kako to biva u ovim igrama, priduživanje familiji donosi moć i prijatelje, ali isto tako veliku obvezu i žrtvu prema istoj. Prethodni protagonisti u Mafia igrama su bili jako dobro napisani, dobro možda ne baš svi, ali Vito i Tommy su jako ikonični. Nadam se da će nas ovaj, poprilično mladi protagonist jednako oduševiti.
Mafia: The Old Country je smještena u početak 20. stoljeća i to na najvećem otoku u Sredozemnom moru, Siciliji. Zanimljivo je kako je ovo prvi Mafia naslov koji se neće odvijati u urbanom okruženju, već ćemo morati jahati konje kako bi mogli putovati. Osim toga, naglasak Mafia: The Old Country će biti na prirodi, što do sada također nije bio slučaj u ovim igrama.
