Igra, smještena u Siciliju s početka 1900-tih, vraća serijal korijenima s linearnijom pričom nalik prvim dvama nastavcima. No, s uvođenjem Free Ride moda igrači će dobiti priliku istraživati svijet i uživati u otvorenijem iskustvu.
Free Ride se prvi put pojavio u Mafia: Definitive Edition, a omogućava vožnju i istraživanje mape, uz razne sporedne misije. Premda nije punokrvni open world poput Mafia 3, dodatak ovog moda ipak nudi određenu dozu slobodu uz koju igrači mogu uživati u talijanskom krajoliku i atmosferi.
Hangar 13 i 2K Games su potvrdili da će mod stići besplatno u budućem ažuriranju, zajedno s novim sadržajem. Uz to su podijeljene i zanimljive statistike, do sada je ispaljeno preko 600 milijuna metaka, a pogođeno je “samo” 185 milijuna protivnika. Nadalje, čak 10 milijardi Dinara je zarađeno i 17 milijuna bandaži je iskorišteno.
